Creighton University Medical Center and Bergan Mercy Medical Center consolidated under one provider number effective 2/7/16.  For verifications for either of these campuses from 2/7/2016 through 6/9/2017, use the ACH-Bergan Mercy Medical Center.

 

On June 9, 2017, the physical hospital campus of Creighton University Medical Center closed and all activities were moved to the Bergan Mercy campus.  It is now called Creighton University Medical Center Bergan Mercy.  Please use CHI Health Creighton University Medical Center Bergan Mercy for verifications after 6/9/2017.
